In one of the biggest surprises, "A.D.", the main antagonist of seasons 6B and 7, is revealed to be Spencer Hastings's identical twin sister, Alex Drake, also played by Troian Bellisario. Spencer and Alex were separated at birth and Alex set out on a quest to destroy Spencer and her friends' lives. In the Pretty Little Liars series finale, season 7, episode 20 "'Til Death Do Us Part", Alex Drake has Spencer captured and tied up in her house. Alex intends to keep Spencer a prisoner indefinitely and continue taking over her life. Mary is a willing participant in capturing Spencer, but she begins to be worried about how violent Alex is willing to get with Spencer. Spencer escapes her imprisonment and duels Alex. Before Alex can kill her twin, the police and Spencer's friends arrive and arrest the Drakes. However, the cop who takes the mother-daughter-pair away looks very similar to Mona's (Janel Parrish) boyfriend.

It's then revealed that Alex and Mary Drake, instead of being in jail, are in , trapped in Mona's doll house and dressed like dolls themselves. Mary says she doesn't think Mona can keep them forever, to which Alex replies that Mary clearly doesn't know Mona. Dolls and dollhouses are significant themes in Pretty Little Liars. The villains of the series, the A-Team, and Alex's A.D. Team, treat the Liars at the center of the story as dolls. They play games with them, treat them like pawns, and manipulate them the way someone would dolls.

Mona's dollhouse, introduced earlier in the series, is a prime example of how moving others, like marionettes, is a core text of the show. Alex, sometimes referred to as "Uber A", in direct contrast to Mona who was at one point just "A", is now a doll in Mona's playroom. Once the chief manipulator, Alex now finds herself at the mercy of another formidable puppet master. Alex and Mary's world of blackmail and lies has finally caught up with them.

Though Alex and Mary Drake are never seen in any of the spinoffs of Pretty Little Liars, their fates are alluded to in the 2019's Pretty Little Liars: The Perfectionists. In this series, Mona has left and moved to Oregon. When asked why, she refers to the "reasons" she was there that had "escaped" her, which could only mean one thing. Later, when Dana Booker (Klea Scott) asks Mona if she's worried about Alex and Mary Drake seeking revenge on her, it all but confirms that the Drakes were indeed able to escape their capture.
