The origin of this name was revealed in Star Wars: The Clone Wars season 7, episode 9 "Old Friends Not Forgotten," which came after its Star Wars debut in Star Wars Rebels. Inspired by her Jedi Master's old communications channel, Ahsoka took on the name to keep her identity a secret, and it became an integral part of Rebellion intelligence.

Cassian, when he earned his rank of captain in the Rebel Alliance, was one of the operatives to take on the Fulcrum codename, doing so most notably as a part of the Alliance's mission to the planet Mustafar. This means that Cassian was actively using a title that originated with Anakin Skywalker, linking the two heroes together.

After being trapped on a desolate planet with Garazeb "Zeb" Orrelios, Kallus was inspired to defect to the Rebellion, though this journey began with him acting as a spy within the Empire's ranks. He is simply one of the many to take on the Fulcrum name, a title in Star Wars that originated with Anakin Skywalker and has since been ed down from his own apprentice to people like Cassian Andor.
