Keipo's relative Chiko tells the envoy to keep the Heart of the Leviathan from Keipo, as it can be used to make a poison that Keipo may use to end his life. Keipo, however, is convinced that he has the Dreamscourge and wants to go out on his own . Both avenues have different moral implications and unique rewards, but it's possible to pick up everything in one go with a hidden choice.

Stopping The Dreamscourge Affliction
If you give Keipo the Heart of the Leviathan, he will use it as a solution to take life into his own hands before the Dreamscourge can. Chiko is upset about this and scolds you, but they don't know the circumstances that led Keipo to this decision. However, Keipo gives you Umpara's Spear, a one-handed melee weapon of Exceptional Quality.

Going Against Keipo's Wishes
If you tell Keipo that you won't let him use the Heart as a poison, then Chiko rewards you with a unique set of armor instead of the spear Keipo gives you. Specifically, Chiko gives you Keipo's Stormdancer armor, which includes the Stormdancer's Scale, Stormdancer's Gloves, Stormdancer's Boots, and Stormdancer's Ring. At face value, giving the Heart to Chiko is definitely the better option, unless you're in desperate need of a spear.

Make Up Your Mind, Avowed: Should I Go To Camp Or Not?
Rarely will you find players complaining about time-skip mechanics, but I really am loathe to miss Avowed's beautiful nighttime scenery.
However, it's possible to get both the spear and the armor if you make the right choices. The key to getting both is reading Keipo's Weathered Journal, which can be found behind a hidden door near the Heart of the Leviathan, and making highly specific dialogue choices when you speak to both Keipo and Chiko. You need to tell Keipo about Umpara last in conversation. Then, tell Chiko about Keipo's sickness and what he wanted. If you do this, you get the spear, armor, and the Heart of Leviathan as a trinket, making it the best outcome in of rewards.

Avowed: Should You Kill Haiako & Katoa Or Reason With Them?
During the Don't Look Down quest in Avowed, you can reason with Haiako & Katoa at the top of the tower - or kill them and take their gear.
Unlike some quests, the consequences of your choice in "Heart of Valor" don't seem to have much impact aside from the rewards that you receive, even if Chiko's scolding feels dire if you give the Heart to Keipo. As a quest centered around a character's right to die, the moral choice could be highly personal, so there's no simple right or wrong answer to the situation. Luckily, you can go about your choices in a way that maximizes the rewards you can pick up, giving you a nice power boost if you need it in Avowed.
