Most of the noteworthy cameos emerge from the Illuminati Council, including the live-action debut of Captain Carter. Other great cameos feature elsewhere in the movie, including both main and obscure characters from Doctor Strange lore. The mid-credits scene introduces a major character from the comics with deep ties to Doctor Strange certain to be a heavy hitter in the MCU's future.

Pizza Poppa

Bruce Campbell reliably cameos in every Sam Raimi movie and he continues the tradition here as "Pizza Poppa." He sells pizza balls in Earth-838 that aren't free, as America Chavez discovers. Pizza Poppa demands she pay but they don't have any money in this universe, so Doctor Strange places a spell on him that forces Pizza Poppa to keep hitting himself.

This goes on for three weeks and recalls Ash from the Evil Dead series and his possessed hand. The payoff comes in the post-credits scene at the end of the movie. Pizza Poppa turns to the camera and says, "It's over" in a very meta moment.

Rintrah

Doctor Strange 2 Trailer Rintrah

Rintrah counts among the best cameos in Doctor Strange in the Multiverse of Madness, even if he isn't the most recognizable. This green goat-like being actually hails from another dimension and appears in the best Doctor Strange comics of the 1980s.

Rintrah appears in Kamar-Taj and helps defend the sacred temple from the Scarlet Witch. He survives the devastating attack and later helps America Chavez learn magic in a scene at the end of the movie.

Ultron

Doctor strange 2 Ultron Drones

The trailer spoiled the appearance of Ultron drones, which triggered speculation they were possibly under the command of a powerful Iron Man variant. The movie makes clear this is actually Ultron himself, as he says 'Ultron orders you to stop.'

This version of Ultron operates through the drones like the Earth-616 version did, though fans don't see a "main" Ultron in this universe. The Scarlet Witch destroys the Ultron drones with relative ease as she rampages through the Illuminati compound.

Captain Marvel

Captain Marvel uses her powers in Doctor Strange in the Multiverse of Madness.

A powerful Captain Marvel variant cameos as part of the Illuminati council. This version of Captain Marvel dismisses the threat of the Scarlet Witch but dies at her hands in a titanic battle in which Wanda seemingly strips Captain Marvel of her power.

Maira Rambeau, the mother of Monica and dear friend of Carol Danvers, functions as the Earth-838 Captain Marvel. In this universe, she must have flown the plane instead of Carol which crashed and unleashed massive cosmic energy to create Captain Marvel.

Captain Carter

Captain Carter fights Scarlet Witch in Doctor Strange 2

Captain Carter ranks as one of What If..? take on the character.

The movie makes clear this isn't that Peggy, though. Her blithe dismissal of the Scarlet Witch's threat — a common feature among the Illuminati — diverges from the more practical approach of the What If..? Captain Carter. Captain Carter dies fighting the Scarlet Witch in a gruesome way.

Black Bolt

Black Bolt appears in the Inhumans TV series.

Black Bolt serves on the council in Earth-838 and features a more comic-accurate look with his signature tuning fork on his forehead. He dies terribly at the hands of Wanda, though Black Bolt likely appears in Earth-616 in the future.

Professor X

Professor X appears in Doctor Strange in the Multiverse of Madness.

Trailers exposed the presence of Professor X in the movie, but he remains one of the best cameos in Doctor Strange in the Multiverse of Madness. Patrick Stewart reprises his role as Professor X, who in Earth-838 leads the Illuminati.

The musical theme of X-Men: The Animated Series accompanies his arrival, and he uses a yellow hoverchair similar to the animated one. He also dies fighting the Scarlet Witch, a sad and shocking end to Stewart's run as Professor X... if it is the end.

Mr. Fantastic

MCU Fantastic Four Movie Multiverse Reed Richards Council Reeds SR

Reed Richards finally debuts in the MCU as a member of the Illuminati Council. Fans cast John Krasinski as Mr. Fantastic in lots of fan art and their wish came true in one of the movie's best cameos. Richards leads the Baxter Foundation in Earth-838 and references his wife and children, presumably the same as in the comics.

Comic book fans know Mr. Fantastic possesses the greatest intellect in the Marvel Universe, but it didn't save him against Wanda. This version dies, but Mr. Fantastic will return in the Fantastic Four movie. The question remains whether Krasinski plays that version or not.

Clea

Clea uses her powers in Marvel Comics.

The best cameo in Doctor Strange in the Multiverse of Madness comes in the mid-credits scene. Charlize Theron appears to take Doctor Strange to the Dark Dimension, warning that he created an incursion that threatens his universe. She plays Clea, a major character from the Marvel Universe.

Clear reigns as the Sorcerer Supreme of the Dark Dimension and is also the niece of the villain Dormammu. She studied magic with Doctor Strange and ultimately became his lover. She currently serves as the Sorcerer Supreme in Earth-616 in the comics, after the death of Stephen Strange.
