Things change dramatically for D. Barrett Cain in season 4. While attempting to save a woman, Cain is hit by an ambulance and spends much of season 4 recovering from a potentially career-ending injury. At the same time, he faces a lawsuit for his improper actions during the COVID-19 pandemic. All this combines for a very humbling season for the egotistical doctor, and he's noticeably less braggadocious after his trials. While some characters like Nicole Nevin left The Resident in dramatic fashion, Barrett Cain left fairly innocuously. A partial reason for this sudden departure was because Morris Chestnut's contract expired after season 4 (via Deadline). Originally, Cain was supposed to return in a recurring capacity, but the showrunners couldn't figure out a way to bring the character back (via TVLine).

One idea included Cain tempting Billie's son, Trevor (Miles Fowler), to pursue work in big pharma. Cain would represent a surgeon beholden to pharma money, and the storyline would explore the dark side of big pharma. Instead of returning as Dr. Cain in The Resident, Chestnut went on to star in the failed TV show Our Kind of People as Raymond Dupont and in the successful miniseries, The Best Man: The Final Chapters as Lance Sullivan. Chestnut is also set to have a big role in Reasonable Doubt as Corey Cash and in Watson as Dr. John Watson.
