One of the more lengthy and intriguing NPC questlines in Fort Faroth in Elden Ring.

Upon entering the Church of Plague, Tarnished will hear Millicent writhing in pain. If the player speaks with her, she will say the same dialogue continuously, warning the protagonist to "leave immediately" because of her miserable condition. From here, there is no other way to interact with her or begin her story. As a result, some Tarnished may be bewildered about what they should do or give her to advance her questline. Fortunately, the answer is close by at a small dilapidated shack southwest of the church.

Along the main southern path leading to Sellia, Town of Sorcery, Tarnished will by a ramshackle hut known as Gowry's Shack. Inside the empty home sits Gowry, an NPC who serves as the key to starting Millicent's Elden Ring. Adventurers can reach Gowry by riding northward from the Southern Aeonia Swamp Bank or ing through the Town of Sorcery by traveling from Sellia Under-Stair. Both routes contain dangerous enemies, so players are advised to choose the path they feel is the quickest and easiest. This is because they will need to make a few trips back and forth between Gowry's hut and Millicent at the Church of Plague.

Speak with Gowry and exhaust his dialogue to be tasked with finding a needle that will cure Millicent's Scarlet Rot. This quest item is acquired by defeating an Elden Ring mini-boss named Commander O'Neil, located southwest of Sellia. After delivering Gowry the needle, he will request that the player give it to Millicent. Return to the Church of Plague to have her use the needle to heal her condition.

Next, go back to Gowry's Shack after reloading at a Site of Grace to find Millicent contemplating her lost memories. Reload again and travel back to the shack to speak with Gowry, who will become a Spell merchant once his dialogue ends. This occurrence indicates that Millicent's questline has begun, and Tarnished can meet her again once they've made it to the Atlus Plateau in Elden Ring.
