Toga and Uraraka have been facing off for a while now, but only came back into focus in the last few chapters. Their conflict has been focused on Uraraka's desperate attempts to talk with Toga, while the latter had been viciously rebuffing every approach. Toga even stabbed Uraraka with a knife, a wound that was bleeding quite badly when she finally managed to get through. In fact, Uraraka was left bleeding out at the end of chapter 394, putting her in a highly dangerous situation. Having gotten her message across to Toga, though, the young villain had a fairly sudden change of heart.

Toga's Story is One of MHA's Most Tragic

Toga lingered near the injured Uraraka before using her blood collecting machine to start pumping new blood into Uraraka to replace what she's lost. By turning into Uraraka, Toga could ensure that the blood would match, and decided to offer up as much as needed, even all of it, if that kept Uraraka alive. Her shift in opinion was a direct result of Uraraka's repeated attempts to get through to her, and she finally managed to do so by complimenting Toga's smile. It may sound shallow, but Toga has always been regarded as a "weird kid" and wasn't on the receiving end of much love, especially from her parents.

For Toga, that may well have been the first time someone she cared about said something nice to her; her parents had treated her exclusively as a weirdo for most of her life, afraid of her attraction to blood. This brief moment of kindness was enough to get Toga to realize that love had to be a two-way street, and that included her own twisted view of what love is. If she's really to love and be loved, Toga would have to give away her blood as readily as she'd take blood from others. It's tragically far, far too late for this realization to help Toga, but it's not too late for Toga to help Uraraka.

While Toga still has a bit of a warped view of what love is, she did finally come to understand that love must be given and not just taken. It goes to highlight that Toga really is still just a kid who's made some truly horrible mistakes, giving her arc one of the more tragic conclusions of the series. The implication seems to be that Toga is sacrificing her life to save Uraraka from an injury she inflicted out of her own hurt, atoning with the only thing she has left to offer. My Hero Academia has brought another villain's life to a close, in turn making Toga one of the series' saddest and most tragic characters.
