Nier's first look is that of a very young silver-haired teenager that is just his standard outfit. Once the player starts the second arc they will acquire Nier's adult look, which is the same character but aged up five years. There are alternate looks for both the young and adult versions of the character that can be unlocked by ing the "4 YoRHa" DLC. With this DLC both outfits will be altered to instead resemble that of the android characters from NieR Automata. In both alternate outfits, NieR will receive a black jacket and black cloth tied around his eyes.

There are two other costumes that the player can get their hands on, but these can only be acquired by making their way through the 15 Nightmares. These optional challenge dungeons have been included as a way for the player to test their combat skills and earn special items. The player will unlock the two other outfits by completing different missions in the 15 Nightmares. Nier's more colorful Kabuki outfit can be obtained by the player completing the fifth level of the first dungeon. The more armored Samurai outfit on the other hand is acquired by completing the fourth level of the second dungeon.
