In season 3 of Peaky Blinders, Tommy and Lizzie have a daughter, named Ruby. She has been rarely seen in the series until season 6, where it is revealed that she has been having visions and repeating the ", to Tommy's horror. Tommy, already convinced that his family is cursed, orders his wife Lizzie to see that a black madonna is put around Ruby's neck. Ruby recovers for a while, but then begins coughing up blood at the end of episode 2, which could be evidence that Tommy's preventative measures might not be enough.

The trailer for Peaky Blinders season 6 shows a funeral that isn't Aunt Polly's, with Lizzie in attendance and very clearly distraught. In the shot, Lizzie is dressed in black along with Charles (the only child of Tommy and Grace,) with a trailer being burned in the background in accordance with Shelby funeral tradition. Ruby is nowhere to be seen in the shot, and so this, along with her illness, makes it likely that the funeral shown is, unfortunately, hers.
Upon telling him about Ruby's mysterious visions, Tommy was visibly shaken and clearly fearing the worst for his daughter. He scrambled instructions to Lizzie regarding the black madonna, as well as ordering Ruby to be kept away from water and horses. This visceral reaction from Tommy, who has in recent times appeared much more calm and collected due to his sobriety, hints at the possible fatality of Ruby's illness. What makes Ruby's fate even more worrisome is the fact that Tommy attempts to Esme at the end of episode 2. Esme is a woman well-acquainted with the supernatural, but she hasn't been seen since series 4. She cursed the Shelby family after the death of her husband, John, thus Tommy's desperation for help is evident, and only makes Ruby's condition appear more severe.
Tommy has already lost his beloved Aunt Polly, who he relied on a lot throughout the course of the show. It crushed him but also lead him to sobriety - however, Ruby's possible fate might not bring about such positive change for him. Tommy's children aren't seen so frequently in the series, but his rash decision to travel home from when Ruby is first ill shows how much he cares for them. The hints regarding Ruby's Peaky Blinders future are made even sadder in conjunction with Tommy's plans to retire his old ways, as he so desperately wants to spend time with his family and finally rest.
